Guidelines for the Recognition, Assessment and Treatment of Pain
NSAIDs are advised against during pregnancy due to teratogenicity risk, while opioids are commonly used with careful monitoring of neonates for depression (reversible with naloxone); alpha-2 agonists and ketamine are avoided or used cautiously for their effects on uterine tone/blood flow. During lactation, a single NSAID dose is considered an acceptable compromise despite limited safety data, and incisional local-anesthetic infiltration is highlighted as a low-risk option.
