Guidelines for the Recognition, Assessment and Treatment of Pain
Gabapentinoids (gabapentin or pregabalin) are recommended as first-line treatment, often combined with an NSAID when inflammation is also suspected; amantadine is noted for osteoarthritis pain refractory to NSAIDs alone, suggesting a neuropathic component. Chronic cases call for multimodal, trial-and-error treatment combining an NSAID with adjuncts (gabapentin, pregabalin, amantadine, amitriptyline); severe acute-on-chronic cases may need hospitalization for IV lidocaine/ketamine with opioids.
