NSAIDs plus an opioid form the core perioperative protocol, with the same NSAID kept consistent pre- and postoperatively; locoregional nerve blocks are recommended in all cases and become mandatory when opioids aren't available. Gabapentin is suggested perioperatively for procedures with nerve manipulation risk (e.g. amputation) to help prevent persistent postoperative pain.
