Guidelines for the Recognition, Assessment and Treatment of Pain
Opioids are first-choice for severe or moderate medical (non-surgical) visceral pain, combined with an NSAID once the patient is hemodynamically stable, with ketamine or lidocaine infusions added for severe cases. For mild-to-moderate pain, an NSAID with or without buprenorphine is suggested — gabapentin is noted as better suited to chronic/neuropathic-component pain than acute cases.
